Media Release
For good and valuable consideration, the receipt of which is hereby acknowledged, I hereby authorize fp Music Academy, LLC (hereafter called “fpMA”) permission to use saved images, saved audio recordings, or saved video (collectively termed “recordings”) taken by a representative of fpMA of myself and/or my immediate family (including children, parents, and siblings) and shared in any and all of its publications, including but not limited to printed, digital, and broadcast publications and electronic media including social media for an unlimited period of time. I understand and agree that any of these recordings will become the exclusive property of fpMA. In addition, I authorize fpMA permission to use images, audio recordings, or video that I voluntarily share, in any and all of its publications, including but not limited to printed, digital, and broadcast publications and electronic media including social media for an unlimited period of time. I understand and agree that I will retain ownership of the original recordings that I voluntarily share, and that fpMA will have the irrevocable right to use of the original recordings. I hereby irrevocably authorize fpMA to edit, alter, copy, exhibit, publish, or distribute any or all of the authorized recordings for purposes of publicizing fpMA or for any other related, lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my recordings appear. The edited or altered copy of any recording will become the exclusive property of fpMA. Additionally, I waive any right to royalties or other compensation arising from or related to the use of the authorized recordings. I hereby hold harmless and release and forever discharge fpMA from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I understand that I am hereby authorizing fpMA to capture recordings of myself and my immediate family (including children, parents, and siblings) during events and recitals that are sponsored by fpMA, and that fpMA may use such recordings. I understand that in the event that I withhold my consent to this Media Release, recordings of myself and my immediate family (including children, parents, and siblings) may be excluded from recordings of such events.

I am at least 18 years of age and am competent to contract in my own name. I have read this release before selecting the option below and I fully understand the contents, meaning, and impact of this release.

fpMA is expressly prohibited from using the authorized recordings in the media listed in the Comments below.
